I learnt at the CARA conference in 2007 from a British presenter that adoption rate refers to "One adoption per one thousand live birth in a geographical location (a country for example)".
Highest adoption rate is currently seen in ETHIOPIA.
Guess which country has the lowest adoption rate in the world? INDIA.
I guess this is not going to get better any time soon as we are nowhere close to controlling our population explosion.
