There are number of documents (seven to be precise) to submit at the time of registration on CARINGS or Child Adoption Resource Information and Guidance System to adopt a child. One such a document is 'Medical Certificate' from a medical practitioner.
Unfortunately, CARA didn't provide a format in their schedules but gave broad guidelines as to what a medical certificate ought to contain. There are three elements that one has to keep in mind while preparing this medical certificate:
Firstly, as far as health is concerned, it has to cover three broad areas namely:
Chronic illnesses
Contagious infections and
Fatal Diseases
It might be a ideal to have a checklist of illnesses/ infections and diseases that your medical practitioner would check out to say that you do not have them. Prospective adoptive parents ought to be free of these above mentioned conditions.
Secondly, medical practitioner may have to state the prospective adoptive parents fitness to adopt a child. I really do not know if this is the job of medical practitioner and some might even resist but to have such a statement to say that you're fit to adopt a child would make things lot less complicated.
Lastly, if you're adopting as a couple, make sure to obtain the medical certificates of both.
